This privacy policy describes what personal information I may collect from you (both online and when you attend an appointment). It also outlines why I collect your information, how I use it, how it is stored and your rights regarding any personal information I may collect. I am committed to the highest level of data privacy standards and patient confidentiality. I only process information that is necessary to provide the best possible care to you.

What personal information do I collect?

I currently collect and process the following information in order to provide the best possible treatment and service to you:

· Personal identifiers, contacts and characteristics (for example; name, phone number, address, email contact details).

· Current and historical medical conditions and treatments relevant to your care

· Contact details of GP and any other relevant health care professionals if it is felt that you need to be referred on for other treatment. This would be discussed with you before doing so.

· When complying with the law – e.g. for a court order in order to provide information for a legal claim. Again this would be with your consent.

Why do I collect this personal information?

I collect information for a number of reasons, for example:

– in order to provide the best possible treatment for you

– in order to be able to contact you

– in order to provide legal information, with your consent (as described above)

– In order to send you marketing information about my services, this is only if you give me the consent to do so.

How do I get the personal information about you?

The personal information I process is provided to me if you:

· contact my service (by phone, via my Facebook page or by email from this website)

· at my appointments with you or if you book an online apointment

or when you call the Spiro Clinic, Ludlow to book into my clinics there.

At no point do I sell your information to other organisations. If I need to contact a third party (such as your GP about a foot concern or to refer you to an Ear, Nose and Throat specialist regarding your ear care) then I will seek your explicit consent.

Under the UK General Data Protection Regulation 2018 (UK GDPR), the lawful bases I rely on for processing your information is:

(a) Your consent (which you may revoke at any time) e.g. signing up for marketing information to be sent to you.

(b) Where it is deemed as necessary for the purposes of provision of ear or foot care e.g. when filling in an assessment form regarding your medical history

(c) If I need to comply with any legal laws e.g. a court order in connection with a legal claim

How I store your personal information:

Your information is securely stored: this is either on paper in a locked cabinet or electronically using ‘Writeupp’ clinical medical management software that is backed up daily, securely encrypted and password protected. Only myself, telephone support staff or the receptionists at the Spiro Clinic, Ludlow have access to your personal identifiable information.

I keep personal information about you for 7 years (for over 18’s) and up until your 21st birthday (for under 18’s). This is in line with UK policy on data protection.

I will then dispose of your confidential information by shredding it and destroying it appropriately.

Your right of access – You have the right to ask me for copies of your personal information.

Your right to rectification – You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.

Your right to erasure – You have the right to ask me to erase your personal information in certain circumstances.

Your right to restriction of processing – You have the right to ask me to restrict the processing of your personal information in certain circumstances.

Your right to object to processing – You have the right to object to the processing of your personal information in certain circumstances.

Your right to data portability – You have the right to ask that I transfer the personal information you have given me to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
